Funded by the Big Lottery, the Access for BAME Elders project aims to enhance physical and menta wellbeing of Black, Asian and Minority Ethnic elders in Lewisham and Southwark, by improving access to mainstream and culturally specific services. Using Person Centred Planning tools, a trained volunteer will work with you to create your unique person centred plan and help you to take the first steps in making it a reality.
Working with your support planner, person centred planning will help you find the confidence and self belief to explore what's important to and for you, and identify the neccessary support available to help you achieve your goals.
Following Joan's recent fall, she was feeling
increasingly lonely as she began to go out less than
she used to. Together with her support planner, Joan
recorded the things she used to do that made her
happy; her good days, bad days and what was
important to and for her. Joan recorded her dreams
and wishes of what she wanted her support plan
to look like. Since setting up her support plan, Joan
now regularly attends church, has joined a singing
group, and visits the hairdressers and her friends.
